The Community Health Centres of Northumberland (CHCN) is a community-governed, not-for-profit organization that provides a comprehensive range of integrated primary health care services and community-based programs that support the physical, mental, and social well-being of individuals and families across Northumberland County. Services are delivered by an interdisciplinary team including physicians, nurse practitioners, nurses, social workers, dietitians, diabetes educators, dental professionals, counselors, health promoters, and community outreach staff.
