Job Summary:
Physical Therapist needed to evaluate and treat patients in an outpatient setting, with occasional coverage in inpatient/critical access and long-term care settings as needed. This role supports functional rehabilitation through individualized plans and consistent clinical documentation.
Responsibilities:

• Evaluate patient needs using appropriate assessment techniques to determine functional limitations.
• Develop and implement personalized treatment plans to improve mobility, strength, and independence.
• Administer therapeutic exercises, manual therapy, and functional training based on clinical findings.
• Educate patients and caregivers on home programs, precautions, and progress toward goals.
