1. Job Role

• The Supervisor oversees a team of case workers responsible for resident care coordination. Develops and implements case management programs, including utilization review, intake or discharge planning, and manages the department as per the regulatory practices and policies.

2. Job Description

• Responsible for the direction, coordination, management, review, implementation and evaluation of resident case management for the organisation in alignment with the organisation's strategies and policies and procedures and directions of higher management.
• Manage and supervise a team of case workers and identify opportunities for team training and skills advancement.
• Manage the safe custody, discipline and needs of the residents in the Home.
