Salary: up to $4900

Working Days & Hours: Mon-Fri office hrs

Location: Chinatown

• Perform health screening such as blood glucose monitoring, measuring of blood pressure, nutritional status and BMI, and make appropriate referrals as necessary.
• Perform nursing care such as administering medication and wound dressing as required.
• Perform initial and 6‑monthly full assessments (set consisting of various assessment forms stipulated) for all clients.
• Assist in the development of individual care plans for all clients.
• Implement and review individual care plans for all clients.
• Plan, conduct and evaluate clients' participation in programme activities and manage accordingly.
• Ensure comfort and safety of all clients.
• Manage and escalate appropriately in the event of unexpected and/or abnormal changes.
