Speech Therapist SLP for Home Health

Overview

FeldCare Connects, an app-based referral network, is seeking an enthusiastic, self-motivated Speech Therapist to deliver quality home healthcare to patients of all ages. This position is for an Independent Contractor to serve Stockton and the surrounding areas.

Responsibilities

• Observe, record, and report the patient's response to treatment and changes to the patient's condition. You determine the plan of care.
• Provide one-on-one treatment and document accordingly. Many clinicians complete documentation during the visit using our Electronic Medical Record system.
• Collaborate with administrative staff for assigning, communication, scheduling, care coordination, and quality assurance support so you can focus on patient care.
• Deliver home healthcare services to patients of all ages in their homes within the assigned coverage area.
