- Develop individualized treatment plans and ensure continuity of care
- Collaborate with multidisciplinary healthcare teams
- Maintain accurate medical records and follow clinical guidelines and best practice
- Provide patient education on disease prevention, lifestyle modifications, and health maintenance
- Perform routine follow-ups and ensure continuity of care for patients
- Identify cases requiring specialist referral and coordinate multidisciplinary care
