Social Worker/Case Worker at Mobile Support Team (COMIT)

SAMH manages the Mobile Support Team (MST), a community‑based allied health‑led team that delivers psycho‑social therapeutic interventions for clients with mental health needs while supporting their caregivers to maintain client wellbeing in the community.

Responsible for the provision of case management services and support for persons in recovery from mental health conditions or possible mental health conditions, and for their caregivers.

• Process referrals, conduct intake assessments and formulate individualised case plans to support clients’ recovery.
• Implement and coordinate follow‑ups with case plans.
• Perform the necessary screening and assessment tools (eg. GAF, Zarit Burden) to facilitate case planning.
• Conduct home visits to clients and caregivers where necessary.
