The Role
We are seeking a motivated and experienced Social Worker to join our Adult Social Care Transitions Team. Our team supports young people aged 16–25 with a wide range of needs as they move from children’s services into adulthood. This is a rewarding opportunity to work alongside young people, families, and partner agencies to deliver timely, person‑centred transition planning that promotes independence and achieves positive outcomes. You will manage a varied and complex caseload, applying your professional judgement within a supportive team environment, and contributing to high‑quality, strengths‑based practice across the service.
