About the Role
To provide social work services to patients and their whanau of Te Whatu Ora- Southern by:
- Undertaking social worker assessments, interventions and evaluations
- Working with the MDT to ensure safe care plans
- Protecting vulnerable patients from harm because of abuse and neglect
- Collaborating community service providers to deliver wrap‑around supports for patients
