Social Worker (Mental Health Treatment Coordinator)

Summary This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application. Responsibilities Serve as a member of a BHIP team as the MHTC, which has primary responsibility for Mental Health (MH) Care Coordination (CC) on the team. Complete MHTC needs assessment as part of Veteran care planning. Complete or review the 5-point screening for MH patients. Participate in care planning to ensure integration of CC interventions within the overall care plan.
