This position offers a unique opportunity to practice Social Work at the front end of Emergency Department care, supporting patients earlier in their ED journey and working proactively with interdisciplinary partners to enhance patient flow, safety, and access to appropriate supports. The successful candidate will work closely with nursing, physicians, Transition Coordinators, and community partners to identify psychosocial needs, facilitate timely interventions, and support connection to community-based services where appropriate.
