The Social Worker II in the Rockyview General Hospital (RGH) Emergency Department provides critical psychosocial support within a high-volume acute care setting. This role serves patients and families presenting with complex social, emotional, safety, and system-related challenges that influence emergency care, patient flow, and safe discharge planning.
The Social Worker II conducts rapid psychosocial assessments, delivers crisis intervention and brief therapeutic support, and facilitates discharge planning for patients across the lifespan. Social work involvement in the Emergency Department plays a vital role in addressing the social determinants driving ED utilization and in supporting timely, appropriate transitions to community, home care, or continuing care services.
