Social Worker (FSC, Admiralty)

The Social Worker (FSC) provides casework, counselling, and community support services to vulnerable families and individuals at the Family Service Centre. The role involves conducting assessments, planning interventions, managing cases, and collaborating with community partners to ensure holistic support for clients in line with social work values and ethics.

• Manage case referrals.
• Manage 30 C&C active cases (mainly Group 2 cases) per quarter.
• Conduct assessment, planning and intervention for cases.
• Conduct case conferences with partners.
• Participate in networking and case management meetings.
• Document and evaluate case outcomes.
• Develop and review appropriate intervention strategies.
• Ensure reports on cases are submitted in a timely manner.
