Social Worker / Case Worker (TOUCH Development &Therapy Services)

Job Summary: The incumbent is responsible for delivering holistic case management services for children and their families through engagement, assessment, intervention, and follow-up. This includes conducting bio-psycho-social assessments, developing and implementing care plans, and working closely with caregivers, educators, and community partners to support the child's development. The role also ensures accurate documentation, effective communication, and coordination of resources to achieve positive outcomes for children and families. Principal Responsibilities and Duties: 1.
