This role supports UNC Health’s Memory Connect program—a virtual, interdisciplinary, primary care–integrated dementia care model—by providing comprehensive social work services to people living with dementia and their caregivers. As a key member of a collaborative team that includes an APP and a Geriatrics physician, the Social Worker (BSW) delivers personalized care planning, caregiver education, behavioral symptom support, and coordination with internal and community resources.
