A Brief OverviewProvide patients and their families, including vulnerable populations, with the psychosocial support needed to cope with chronic, acute, or terminal illnesses. Services address their social, emotional, and financial needs. Services provided include clinical assessment, crisis intervention, brief therapy, case consultation with the healthcare team and collaboration with other professionals in the community. These services advise family caregivers, provide patient education and counseling, and make necessary referrals to community resources for continuity of care.What you will do
