Job Description
Social work resource for team members and care coordination program.
Provide guidance through challenging situations.
Promote problem-solving/critical thinking skills among team members to be utilized upon new challenges/barriers.
Assist with interviewing and hiring staff members who will enhance the care coordination program’s ability to meet its goals and objectives.
Coordinating orientation and training for newly hired staff. through personal mentoring or assigning a mentor, and monitoring the new staff member’s progress towards established expectations.
