Job summary
An exciting 12-month opportunity has arisen within CNWL Foundation Trust's Rapid Access Services (RAS) team. This role offers the chance to develop advanced clinical and leadership skills within a dynamic multidisciplinary service delivering integrated community healthcare.

RAS works closely with acute trusts, social care and third-sector partners to provide acute care in the community, helping people remain safely at home, preventing unnecessary hospital admissions and supporting timely discharge.

The service includes three pathways: Discharge to Assess (D2A), supporting safe hospital discharge through urgent home-based therapy; Urgent Community Response (UCR), providing rapid assessment and intervention to prevent admission; and Camden Hospital at Home (H@H), a Consultant-led service delivering hospital-level care in patients' homes.
