Senior Nurse: Chronic Disease Mgmt & Education

NHG Health invites applications for the Staff Nurse / Senior Staff Nurse (Chronic Disease Management) role in Singapore. You will develop and provide independent nurse-led services in chronic disease management, health promotion and preventive care within National Healthcare Group Polyclinics.

The position requires post-basic qualification in Diabetes Management and Education, Gerontology or Chronic Disease Management, and registration with the Singapore Nursing Board.
