Overview

The Population Health Management (PHM) Directorate has been established to support daily clinical management to become more systematic and targeted; to ensure NHG Health is more informed by clinical, operational and demographic data as Regional Health Manager for Central and North Singapore.

This role facilitates change across NHG Health at population, provider, place, and resident level—ensuring that we address not just the clinical and preventative aspects of care but also the social, environmental, and behavioral determinants of health.

We aim to add 15M health years for our residents.

Major Duties and Responsibilities

• Drive population profiling and asset mapping to inform system planning and care delivery.
• Use population health data to draw insights for strategic planning.
• Maintain the Knowledge Repository on Population Health SharePoint.
