Senior Community Nurse in Aberdeen

Join Aberdeen City HSCP as a Senior Community Nurse and provide exceptional end-of-life care. This role focuses on person-centered nursing, triage, and managing unscheduled referrals from 18:00 to 00:00.
The Senior Community Nurse will enhance patient care in their homes while supporting Band 5 and Band 3 colleagues. You’ll play an integral role in avoiding unnecessary admissions by triaging referrals from GMED and NHS 24. Your responsibilities will also include end-of-life care and supporting the District Nurse in team management duties.
Key Responsibilities:
• Deliver person-centered nursing care to patients at home

Requirements:
