As the first point of contact, you will guide members through the enrollment process, assess eligibility, identify unmet social and health needs, and connect individuals to appropriate community and healthcare resources.

Key Responsibilities

Screening & Intake

• Conduct eligibility screenings and assessments for SCN services.

• Collect demographic, medical, insurance, and social needs information.

• Identify barriers to care, including housing instability, food insecurity, transportation challenges, unemployment, behavioral health concerns, and chronic health conditions.
