Position Description

Elevate is looking for a LPN or RN to be part of the Assertive Community Treatment (ACT) in Southeast Iowa. The community nurse will assess and monitor ongoing medical, psychiatric and social needs. They will coordinate with the psychiatric nurse practitioner as well as other providers in the ACT team and any external supports and providers. This position does require travel through the service area: Henry, Des Moines, Lee, Louisa, and Washington.
