RN Home Care Supervisor - Per Diem - Rochester

Liberty Home Care is looking for a motivated Registered Nurse to join our Licensed Home Care team as an RN Field Supervisor!

Schedule: 2 days per week 8:30am-4:30pm


Duties & Responsibilities:

+ Provide assessment of newly referred patients and assess health needs in home care setting

+ Work with patient to create a patient centered plan of care

+ Reassess patient needs for services on a continual basis and discuss any changes in the plan of treatment with the patient physician/family

+ Prepare written documentation and summaries of the patient’s progress as needed for the continuation of home health services

+ Provide supervision and training of PCA/HHA staff

+ Assist with the on-call rotation (stipend provided)

Qualifications:

+ NYS Licensed Registered Professional Nurse

+ Minimum of Associate's Degree required
