RN Case Manager — Impactful Home Health in Colorado

FreedomCare is seeking a compassionate and motivated RN Case Manager to join our Colorado team. In this role you will build meaningful relationships with patients and caregivers, use clinical judgment to guide care, and help people remain safe and supported in their homes.

You will assess patient needs, develop individualized plans of care, monitor condition changes, educate and support families, coordinate services with physicians and care teams, and advocate for safe, person-centered home care
