Remote LPN Care Coordinator - Chronic Disease Telehealth

Evergreen Nephrology is seeking a remote Licensed Practical Nurse (LPN) or LVN to perform monthly CCM touchpoints, assess symptoms, and collaborate with care teams. You will document interactions in EHR/ CCM platforms and ensure engagement with patients and caregivers.

The role requires 1–2 years of ambulatory or care coordination experience, strong clinical judgment, and the ability to work from a private home workspace with reliable internet. Flexibility for remote work is essential.
