As a key member of the System Navigation Team, the Registered Social Worker delivers essential services, including psychosocial assessments and collaborative care planning. This full-time position requires strong engagement skills to assist clients in navigating health, housing, and financial services. You'll work closely with various interdisciplinary teams and community partners to enhance client access and support through tailored interventions.
