Enhance senior care as a Registered Practical Nurse at Baycrest Terraces Health Centre. This temporary full-time role involves community health assessments and supportive care for seniors.
The Baycrest Terraces Health Centre serves around 200 seniors, focusing on individual assessment and collaborative care alongside a multidisciplinary team. You will implement and evaluate health promotion programs tailored to residents’ specific needs while providing urgent care and monitoring ongoing health challenges.
Key Responsibilities:
• Independently assess community health needs for seniors
• Develop and implement targeted health promotion programs
• Triage walk-ins and phone inquiries effectively
• Provide urgent and emergency care as necessary
• Support Physicians’ clinics with care coordination
Requirements:
• Active registration with the College of Nurses of Ontario
• Current Basic Cardiac Life Support (BCLS) Level C certification
