Registered Nurse Clinical Coordinator

Summary The Community Care RN Clinical Coordinator utilizes comprehensive clinical practice, case management, and utilization review principles to assess, triage, and plan the appropriate level of care and to ensure compliance with established guidelines resulting in approvals, denials, and coordination of care for individuals referred within the Community Care Network (CCN) or other VHA program as applicable. Responsibilities Major duties: Conducts clinical reviews to interpret and evaluate complex data directly related to diagnosis, condition, and treatment plan to assure appropriate medical necessity, care coordination, and timely utilization of CCN health care services.
