Job Description

Facilitates the patient's hospitalization from admission through discharge. Coordinates with physicians, other nurses and social workers and other disciplines within the care team, including outside agencies, to expedite the appropriateness, effectiveness and timeliness of care.

Job Responsibility

+ Coordinate care with internal and external providers
+ Actively participate with a multidisciplinary team
