**Public Health Nurse II – Adult and Aging Division**
The Department of Family Services (DFS) is seeking a **Public Health Nurse II** to support older adults, adults with disabilities, children, and caregivers through assessment, case management, care coordination, and Long-Term Services and Supports (LTSS) screening. This position conducts home and field visits, evaluates client health and safety needs, determines eligibility for Medicaid-funded LTSS programs, develops care plans, coordinates services, and provides health education and crisis intervention.
The Public Health Nurse II works collaboratively with multidisciplinary teams while maintaining a person-centered and culturally responsive approach. Responsibilities include managing a caseload, completing comprehensive assessments, making referrals to community resources, conducting outreach and health promotion activities, participating in trainings and meetings, and mentoring staff as needed.
