**Job Profile Summary**

​This role focuses on safeguarding and promoting the welfare of patients and can provide services if necessary. In addition, this role focuses on performing the following Community Benefits & Community Health Initiatives duties: commitment to improving the health and wellbeing of, and to reducing health disparities and inequities in, the communities we serve; seeking to identify current and emerging health needs, collaborate with community partners, facilitate culturally and linguistically appropriate health services and resources, and address community health and social determinant of health needs. A professional individual contributor role that may direct the work of other lower level professionals or manage processes and programs.
