Practice Transformation Project Specialist (Hybrid/Jackson, MI) - Mosaic CIN

GENERAL SUMMARY:

The Practice Transformation Project Specialist coordinates high-impact transformation projects and performance reports to ensure successful implementation of workflows, procedures and project goal accomplishment to consistently achieve the Triple Aim goals of improved health outcomes, improved experience of care and improved per capita cost of care. In addition, the Practice Transformation Project Specialist will function as the liaison between post-acute facilities and agencies and the health system.

PRINCIPLE DUTIES AND RESPONSIBILITIES:

+ Partners with leadership and team members to identify and support standard improvement activities across post-acute providers as appropriate to improve network-wide performance.

+ Serves as team lead on various transformation projects related to payor initiatives and the triple aim.
