Summary The Eastern Kansas Health Care System is seeking to hire a Licensed Practical Nurse's to serve our Veterans at the Leavenworth Community Based Outpatient Clinic. Responsibilities Duties to include but not limited to the following: Performs assigned duties in a manner that promotes quality health care, positive customer service and the maintenance of an environment committed to serving the Veteran. Complete phlebotomy duties Assist in the development and revision of the patient-driven holistic life care plan with a focus on health promotion and disease prevention. Provide care to a culturally diverse population with varying levels of complexity. Demonstrates active collaboration, communication, and problem solving with core team (Patient, Primary Care Provider, Registered Nurse, and Medical Support Assistant) and expanded team(family/caregiver, internal and external community-based services who may be involved in providing care to the patient).
