Physiotherapist (West)

Job Description:

• Plan and run physiotherapy rehab services in the TCF (in coordination with the Rehab Manager).
• Conduct assessments for patients during admission and periodic reviews.
• Institute an appropriate and effective treatment.
• Participate in multidisciplinary case discussions with the TCF care team.
• Contribute to formulation and follow-up of patients' individual care plan (ICP) to ensure holistic health and care coverage.
• Plan and conduct group therapy programmes and activities for patients.
• Take care of proper documentation of therapy records.
• Understand dementia issues and know how to manage patients who display behavioural problems (associate with onset of dementia)

Job Requirements and Qualifications:

• Bachelor degree or Diploma in Physiotherapy.
