Physical Therapist - In-Home Outpatient (Older Adult Population)

Position Summary:
The Physical Therapist provides outpatient rehabilitation services in an in-home setting for the older adult population. Services are delivered in patients' private residences or within independent living facilities, allowing therapy to occur in the most functional environment-the patient's home.
The Physical Therapist assesses patient needs and develops, implements, and delivers individualized physical therapy programs to facilitate rehabilitation.
