As a Physical Therapist Assistant I, you will work under the supervision of a licensed Physical Therapist to execute treatment plans for homebound patients. Key Responsibilities

• Implement treatment plans as directed by the supervising Physical Therapist, ensuring adherence to frequency and duration specifications.
• Document patient progress, strength, ROM, balance, and functional mobility using objective measures.
• Communicate effectively with patients, caregivers, physicians, and the rehab team to coordinate care.
• Conduct discharge planning and make referrals for ongoing services as needed.
