Pharmacist (Primary Care) - Fairfield Surgery

Fairfield Surgery is looking for a motivated individual to join our Team. The role will include medicine reconciliations from hospital discharges, undertaking medication reviews and providing a key role in DOAC clinics. Assisting with relevant QI projects and providing advice to the Clinical Team., Medicines reconciliation, repeat reauthorisations, clinical medication reviews, patient queries, DOAC clinics (if appropriate).

Fairfield Surgery is a successful urban Practice in Llanelli Town Centre, serving a population of almost 8,000 patients. The Practice is located in beautiful Carmarthenshire with outstanding coastal and country scenery very close. We are also very near the M4 corridor.
