3-Month Contract Role with Local Authority Summary The Personal Advisor will provide high-quality support, guidance, and practical assistance to care leavers and young people transitioning into adulthood. Working within the Leaving Care Team, the postholder will manage a diverse and complex caseload, including former and current asylum-seeking young people, individuals with unresolved immigration status, and young people with disabilities. Responsibilities Manage a complex and diverse caseload of care leavers and young people aged 1625. Establish positive and trusting relationships with young people, supporting them to identify and achieve personal goals. Undertake comprehensive needs assessments and develop outcome-focused support plans that meet individual needs. Prepare, review, and maintain Pathway Plans in accordance with statutory guidance, ensuring plans are SMART, person-centred, and focused on independence.
