Job Summary
The Perinatal Community Care Co-ordinator is a Band 6 clinician working within the Leeds Perinatal Mental Health Service, providing specialist assessment, treatment, care co-ordination and support to women experiencing acute moderate to severe perinatal mental health difficulties. The post holder acts as an autonomous practitioner managing a complex caseload of mothers and infants, supporting women and their families in the community, facilitating discharge from inpatient services, and helping to prevent hospital admission where possible. The role involves multi-agency working with health, social care and third-sector partners, safeguarding children, delivering evidence-based interventions, participating in regional duty systems, supervising junior staff and students, and contributing to service development across Leeds and the wider Yorkshire and Humber region.
