Pediatric Care Manager RN

**Description**



The RN Care Manager is an experienced Registered Nurse who serves as a core member of a multidisciplinary team, facilitating communication and coordination of patients' healthcare needs among primary care providers, specialty providers, healthcare facilities, community resources, and payers. The RN Care Manager is responsible for coordinating and supporting care for pediatric patients with complex medical and/or psychosocial needs within pediatric clinics, while overseeing transitions of care and facilitating referrals to specialty services, clinically based programs, and community resources.



Through comprehensive care management, the RN Care Manager assesses patient and family needs, develops individualized care plans, promotes patient and family engagement and self-management, identifies and addresses barriers to care, and monitors progress toward established health goals.
