SUMMARY OF RESPONSIBILITIES:

Under the direct supervision of the Integrated Service Program Manager, Patient Navigator supports community members and families seeking health, social, and community services in primarily in Quebec and at times in Ontario. The role focuses on reducing barriers to care, improving access to services, and promoting positive health and social outcomes.
