Patient Navigator in Quebec and Ontario

Enhance care access as a Patient Navigator with Clinical Care Services. This full-time position focuses on bridging health and social services for community members in Quebec and Ontario.
Reporting to the Integrated Service Program Manager, the Patient Navigator plays a crucial role in improving service access and promoting health outcomes. You will assist individuals navigating complex systems, particularly addressing French language barriers. Utilizing your experience in community practice, you will identify service gaps and collaborate with providers to implement effective solutions.
