• Provide palliative & end-of-life psycho-social support for the elderly and their families
• Make financial assessments & casework counselling through home visits team with other professionals to provide a holistic, multi-disciplinary approach to elderly beneficiaries
• Liaise with community service organisations and agencies to bring all resources to bear in palliative care.

• A degree in social work or recognised post-graduate diploma in social work from an accredited university.
• Be a registered medical social worker with the Singapore Association of Social Workers.
