We are seeking a skilled and compassionate Occupational Therapy Assistant to join the CHI Franciscan Home Health team. The OTA will work collaboratively with the OT and other interdisciplinary team members to plan, implement and evaluate occupational therapy services for patients in their homes.
+ Performs visits as prescribed in the physician’s plan of treatment and in compliance with state and federal guidelines.
+ Provides patient care education to patient and other caregivers.
+ Uses skills to monitor patient status, respond to treatment and care provided. Reports changes in patient condition to appropriate personnel and the attending physician.
