As a member of a specialized interprofessional team, the Social Worker/Occupational Therapist leads the team in the engagement and partnership with individuals and their family members or their natural supports, in the treatment/service planning process, and in individual and/or multiple family support therapy. In addition, expertise is provided with respect to entitlements, advocacy and working the system. Depending on the type of referral, responsibilities may include developing and implementing individualized treatment and recovery plans, service coordination, medication management, activities of daily living, and social engagement.
