As a Nurse Practitioner (VSP), you will apply your advanced education and clinical competencies to achieve optimal patient outcomes. You will work on problems of diverse scope and complexity ranging from moderate to substantial.
You will work as a variable service provider NP/PA (APP) role with an average of 20-30 hours per week for a 6-month period. You will work in a remote environment, reporting to our Associate Director, Advanced practice Provider. There are no benefits tied to this position.
You will spend your day performing the following duties:
+ Conducting virtual (audio and video) visits with CenterWell patients to care for chronic conditions, discuss open care gaps, and connect patients back to an in-person PCP.
+ Address acute and chronic conditions and preventive care needs. Creating and registering, and documenting a patient record within Athena
