The Nurse Practitioner with the Specialized Geriatric Clinic is responsible for developing collaborative care and treatment plans within respective scope of practice in collaboration with patients, care partners and specialized health team members. The NP will facilitate comprehensive assessment, consultation, system navigation and coordinating of complex care needs in the spirit of optimizing individual functional independence and overall well‑being of older adults.

QUALIFICATIONS

Certification/Experience/Knowledge:

• Current registration with the College of Nurses of Ontario (CNO) as a Registered Nurse in the Extended Class (RN (EC)) and with a specialty certificate in either Adult or Primary Health Care required.
