As a Nurse Practitioner (NP) or Physician Assistant (PA) at Conviva Senior Primary Care, you will be part of a collaborative, multidisciplinary team that includes physicians, pharmacists, care coaches, behavioral health specialists, and more—all working together to deliver personalized, high-quality care.
**In this role, you will work onsite at our Jourdanton clinic. You will report to our Associate Medical Director.**
