Nurse Practitioner - Home Based Primary Care

Summary This position is located at the Union Grove VA Clinic in Union Grove, Wisconsin. The Advanced Practice Nurse (APN) Certified Nurse Practitioner (NP) practices with the Home Based Primary Care (HBPC) Service to provide a full range of services emphasizing quality, timeliness, and the advancement of the Veterans Affairs (VA) mission. The NP travels to the Veterans home to provide advanced clinical practice, consultation, and management in collaboration with the interdisciplinary team. Responsibilities Major duties and responsibilities of the position: Independently manage a panel of patients with acute and chronic health conditions with an emphasis on health promotion and disease prevention using evidence-based practice. Develop and implement an individualized plan to achieve optimal population health and disease management. Demonstrate leadership and works collaboratively with the interdisciplinary team.
